ОПРЕДЕЛЕНИЕ ФАКТОРОВ ОКРУЖАЮЩЕЙ СРЕДЫ, СВЯЗАННЫХ С ВОСПАЛИТЕЛЬНЫМ ЗАБОЛЕВАНИЕМ КИШЕЧНИКА В КАЗАХСТАНЕ: ИССЛЕДОВАНИЕ «СЛУЧАЙ-КОНТРОЛЬ»
Введение. Исследований, посвященных изучению влияния факторов окружающей среды на риск развития воспалительных заболеваний кишечника (ВЗК) в странах западного мира очень много, но аналогичные исследования в Казахстане до настоящего времени не проводились.
Цель исследования: изучить факторы риска окружающей среды связанные с ВЗК среди населения Казахстана.
Материалы и методы исследования. Исследование «случай-контроль» проводилось на базе научно-исследовательского института кардиологии и внутренних болезней, с применением метода анкетирования для выяснения ассоциаций факторов риска с ВЗК. Опрос проводился с октября 2019 года по январь 2020 года. В исследовании приняли участие 65 пациентов с ВЗК и 173 респондентов без данной патологии. Для выявления ассоциаций между факторами риска и наличием ВЗК использовалась процедура логистической регрессии, оценочным параметром выступил показатель отношения шансов (ОШ) с 95% доверительным интервалом (ДИ).
Результаты. Факторами, ассоциированными с повышением шансов ВЗК были нерегулярный прием пищи (р=0,043; ОШ=3,61 [95% ДИ: 1,04-12,51]), потребление рыбы и морских продуктов (р=0,000; ОШ=15,77 [95% ДИ: 4,56-54,59]), потребление замороженных полуфабрикатов (р=0,018; ОШ=4,62 [95% ДИ: 1,3-16,4]), питание, основанное на мясных блюдах (р=0,029; ОШ=3,2 [95% ДИ: 1,13-9,2]), потребление НПВС, кроме аспирина (р=0,031; ОШ=3,79 [1,13-12,69]) и курение (р=0,008; ОШ=4,93 [95% ДИ: 1,52-15,98]). В то время как казахская национальность (р=0,011; ОШ=0,18 [95% ДИ: 0,05-0,68]), потребление молока (р=0,000; ОШ=0,08 [95% ДИ: 0,02-0,31]), потребление алкогольных напитков (р=0,000; ОШ=0,04 [95% ДИ: 0,01-0,16]), перенесенные детские вирусные инфекции (р=0,042; ОШ=0,52 [95% ДИ: 0,27-0,98]) являлись протективными факторами при ВЗК.
Выводы. Наше исследование установило, что среда играет важную роль в модулировании последующего риска развития ВЗК.
