АЛЛЕРГЕНГЕ ТӘН ИММУНОТЕРАПИЯ ТИІМДІЛІГІНІҢ ИММУНОЛОГИЯЛЫҚ АСПЕКТІЛЕРІ. ӘДЕБИЕТКЕ ШОЛУ
Кіріспе: Маусымдық аллергиялық аурулар қазіргі уақытта ғаламдық медициналық және әлеуметтік проблема болып табылады. Бүгінгі күні аллергиялық ауруларды емдеудің ең тиімді әдісі - аллергенге тән иммунотерапия. Бұл емдеу әдісі аллергиялық процестің барлық патогенетикалық байланыстарына әсер етеді және емдеу курстарын аяқтағаннан кейін ұзақ профилактикалық әсерге ие.
Мақсаты: Әдебиеттерге сәйкес маусымдық аллергиялық аурулары бар науқастарда аллергенге тән иммунотерапияның иммунологиялық механизмдерін қарастыру.
Іздеу стратегиясы: Дәлелді медицинаның ғылыми мәліметтер базасында (PubMed, Scopus, Ebscohost, Medline, Сохран кітапханасы, SpringerLink, Web of Science) және электронды ғылыми кітапханаларда (Google академиясы, елдердің медициналық мерзімді басылымдарының веб-сайттары) жүргізілді. ТМД, e-library.ru). 2008 жылдан бастап 2020 жылға дейін іздеу тереңдігі. Деректер базасында зерттеу мәтініне сәйкес таңдалған көптеген әдеби көздер анықталды. 119 әдеби көздің ішінен 102 басылым іріктеліп алынды. Қосу критерийлері: осы мақаланың аналитикалық материалы ретінде рандомизацияланған және когорттық зерттеулер туралы есептер таңдалды; мета-анализдер және жүйелік шолулар; толық мәтінді және көпшілікке қол жетімді басылымдар; Орыс және ағылшын тілдеріндегі статистикалық расталған тұжырымдары бар мақалалар. Шығару критерийлері дәлелдемелері жоқ төмен сапалы әдістемелік мақалалар болды.
Нәтижелер: шолуда ең маңызды клиникалық зерттеулердің нәтижелері және аллергенге тән иммунотерапия әртүрлі схемалары талданды. Аллергенге тән иммунотерапия әсері келесі негізгі иммунологиялық механизмдер арқылы жүзеге асырылады: 2 типті Т-көмекшілерінен иммунологиялық реакцияның 1 типтегі Т-гепаратқа өтуін бастау және қолдау; бұл механизмді дамытуда негізгі рөлді T клеткалары атқарады, олар иммуносупрессивті цитокиндер интерлейкин-10 шығарады және G4 иммуноглобулиндерінің өндірілуіне ықпал ететін өсу факторын өзгертеді. Осы процестің негізінде иммуноглобулин Е мен иммуноглобулин G4 арасындағы тепе-теңдік өзгерісі аллергенге тән иммунотерапияны сәтті өткізудің түбегейлі маңызды шарты болып саналатын және ұзақ мерзімді клиникалық төзімділікті қамтамасыз ететін соңғысының өндірісін арттыру бағытында анықталады.
