ВЛИЯНИЕ ЭРИТРОЦИТАРНЫХ ИЗОАНТИГЕНОВ НА РАЗВИТИЕ ЦИРРОЗА ПЕЧЕНИ
Введение. Высокие показатели распространенности и летальности циррозов печени, их поздняя клиническая манифестация, недостатки биопсии печени обусловливают актуальность нашего исследования
Цель. Изучить изоантигенный профиль эритроцитов у пациентов с циррозами печени в зависимости от национальной принадлежности пациента
Материалы и методы. Нами обследовано 140 пациентов с достоверно установленным диагнозом цирроза печени и 272 условно здоровых участника казахской и русской национальности. Проводилось определение фенотипов эритроцитов венозной крови систем АВ0, MNS, P1, CDE. Для сравнения частот фенотипических проявлений использовался χ2 с определением силы ассоциации путем вычисления отношения шансов (ОШ) c расчетом 95% доверительного интервала
Результаты. У пациентов казахской национальности частота встречаемости фенотипа 0(I) значительно выше у больных циррозом печени (ОШ=1,917, 95% ДИ 1,16-3,15, p<0,01) по сравнению с контролем. Также частота встречаемости фенотипов MN (ОШ=2,86, 95% ДИ 1,74-4,70, p<0,01) , D+ (ОШ=2,73, 95% ДИ 1,1- 6,86, p<0,05), СС (ОШ=2,15, 95% ДИ 1,31 - 3,53, p<0,01) и ее (ОШ=1,85, 95% ДИ 1,1 - 3,15, p<0,05) была выше по сравнению с контрольной группой. Среди больных циррозом печени русской национальности шанс обнаружения фенотипа А(II) выше в сравнении с другими фенотипами (ОШ=2,653, 95% ДИ 1,063-6,622, р<0,05), с более высоким шансом обнаружения ММ фенотипа (ОШ=2,879, 95% ДИ 1,200-6,907), фенотипа Сс (ОШ=6,968, 95% ДИ 2,581-18,812) и антигена Р1 (ОШ=3,352, 95% ДИ 1,383-8,126).
Выводы. Таким образом, наше исследование показало, что изоантигены 0(I), MN, D+, CC, ee ассоциированы с возникновением циррозов печени в казахской популяции, а фенотипы А(II), ММ, Сс, P1 ассоциируются с развитием цирроза печени у пациентов русской национальности
