PRINCIPLES OF DIET THERAPY IN FOOD PLANNING FOR TYPE 1 DIABETES MELLITUS IN CHILDREN
Introduction. Type 1 diabetes mellitus (DM) is a severe chronic disease characterized by the development of specific complications that cause early disability and a decrease in the life expectancy of patients. The main damaging factor in the development of diabetes complications is chronic hyperglycemia and concomitant metabolic changes: hyperlipidemia, increased lipid peroxidation processes, increased glycosaminoglycan synthesis, activation of the renin-angiotensin-aldosterone system, etc.
Purpose: to analyze the literature on the assessment of the effect of diet therapy on the normalization of metabolic shifts in type 1 diabetes mellitus in children.
Search strategy: The search for sources was carried out in the databases PubMed, The Сochrane Library, Scopus, elibrary. The search depth was 10 years from 2011 to 2021. Inclusion criteria: data from randomized, cohort epidemiological studies, systematic review and meta-analysis, articles in English and Russian. Exclusion criteria: materials without evidence, newspaper articles. In the course of the search, 87 sources were found, of which 43 sources were selected and analyzed for a more detailed study.
Results: The analysis of the relationship between chronic hyperglycemia and the development of complications of diabetes mellitus was carried out. It has been established that the processes of assimilation of carbohydrates depend on many factors, which ultimately determine the differences in the glycemic potential of different products; the role of insulin therapy with ultrashort drugs in the prevention of early complications of diabetes mellitus in children has been assessed.
Conclusions: The data indicate a decrease in postprandial glycemia under the influence of ultrashort insulin preparations in children with type 1 diabetes mellitus. Their use seems to be an effective method of preventing early complications in patients.
