ANXIETY-DEPRESSIVE DISORDERS IN CHRONIC VIRAL HEPATITIS. LITERATURE REVIEW
Introduction. Chronic viral hepatitis has a wide range of manifestations, ranging from an asymptomatic course of the disease with the possibility of progression to the development of liver cirrhosis and hepatocellular carcinoma. Currently, chronic viral hepatitis is recognized as a systemic disease characterized by a variety of extrahepatic manifestations, such as depression, anxiety, pathological fatigue, irritability, anxiety, insomnia, as well as increased sensitivity to pain. Purpose: to systematize and evaluate the development of anxiety-depressive disorders in patients with chronic viral hepatitis according to modern literature. Material and methods. The search for sources in the databases PubMed, The Russian Library, Scopus, elibrary was carried out. The search depth was 15 years from 2006 to 2021. Inclusion criteria: reports on randomized and cohort studies, meta-analyses and systematic reviews; articles in English and Russian. Exclusion criteria: materials that do not have an evidence base, newspaper articles. Results and discussions. The article presents an analytical review of the literature data on the problem of anxiety and depressive disorders in patients with chronic viral hepatitis. According to the literature review, the prevalence of anxiety and depression in this category of patients varies from 37 to 83%, while there is a relatively high frequency of clinically significant anxiety and depression, and the suicidal risk increases with the transformation of chronic viral hepatitis into cirrhosis of the liver. Conclusions. It was found that in patients with chronic viral hepatitis, changes in the mental status are detected, the severity of which depends on the clinical, laboratory and morphological characteristics, the duration and activity of the process. It is confirmed that timely correction of anxiety-depressive disorders and prevention of disease progression in the early stages can be potentially useful both for improving the psychological well-being of patients with chronic viral hepatitis, and for increasing their adherence to treatment.
