РОЛЬ УСТРОЙСТВ ДЛЯ ФИКСАЦИИ ПОВРЕЖДЕНИЙ ТАЗА НА ДОГОСПИТАЛЬНОМ И ГОСПИТАЛЬНОМ ЭТАПАХ НЕОТЛОЖНОЙ МЕДИЦИНСКОЙ ПОМОЩИ. ОБЗОР ЛИТЕРАТУРЫ

Введение. В научной сфере имеется тенденция к проведению исследований в области неотложной медицины [28, 39]. Травмы таза занимают одно из лидирующих мест среди проблем травматологии и неотложной медицины. Вопрос о выборе лучшего и эффективного способа иммобилизации при повреждениях таза до сих пор остается актуальным.

Цель. Анализ литературных данных о роли устройств для временной и постоянной фиксации повреждений таза на догоспитальном и госпитальном этапах неотложной медицинской помощи.

Методы исследования. В процессе поиска литературы были использованы следующие поисковые системы: Pubmed, Google Академия. Глубина поиска была не ограничена. Мы использовали следующие стратегии поиска в PubMed (MeSH Terms: epidemiology; pelvis; fractures bone; MeSH Terms: equipment and supplies; pelvis; immobilization). Критерии включения публикаций в обзор: публикации, находящиеся в полнотекстовом доступе, на русском и английском языках, несущие статистически выверенные выводы. Критерии исключения: резюме докладов, газетные публикации, личные сообщения. Всего было найдено источников 5457, из которых для последующего анализа отобраны 76.

Результаты. Стабилизация таза осуществлялась долгое время лишь инвазивными методиками, внешними фиксаторами и С-зажимом (C-clamp) [27, 41, 44, 73, 74]. Согласно результатам систематического обзора, охватывающего публикации на английском языке с 1999 года по 2015 год, и др. авторам, эффективными в настоящее время являются неинвазивные тазовые компрессионные устройства [4, 19, 59, 64]. Из-за простоты их применения и возможности контролировать уровень сжатия компрессионные пояса являются предпочтительным методом ранней иммобилизации [9].

Выводы. Актуальным в наше время становится использование неинвазивных тазовых компрессионных устройств. Необходимы проспективные клинические исследования для оценки их клинической эффективности и безопасности.
