РЕВМАТОИДНЫЙ АРТРИТ И ФЕРТИЛЬНОСТЬ: ОБЗОР ЛИТЕРАТУРЫ
Актуальность: Ревматоидный артрит - хроническое воспалительное аутоиммунное заболевание, приводящее к инвалидности, которое встречается у женщин, преимущественно, детородного возраста в три раза чаще, чем мужчин. К сожалению, при этом вопрос о деторождении может быть связан не только с активностью болезни, но и наличием комплексной медицинской помощи и знанием фармакологических методов лечения. Поэтому многие женщины изо всех сил пытаются найти адекватную информацию о планировании беременности, родов и лактации.
Доступность и расширенный выбор фармакотерапии способны поддержать оптимальный уровень течения заболевания до зачатия и улучшить физическое состояние, чтобы успешно преодолевать проблемы, но требуют детального понимания их рисков и безопасности при беременности и грудном вскармливании.
Цель: поиск и обзор релевантных источников по вопросу фертильности у больных ревматоидным артритом.
Стратегия поиска: поиск и обзор литературы был осуществлен в базах данных PubMed (MEDLINE) глубиной 20 лет по ключевым словам. Критерии включения: отчеты рандомизированных и когортных исследований, проведенных на больших популяциях, обзоры, так же публикации с чётко сформулированными и статистически доказанными выводами. С целью проведения литературного обзора изучалась литература на английском языке. Критерии исключения: статьи, описывающие единичные случаи, резюме докладов, личные сообщения и газетные публикации, тезисы, личные сообщения.
Результаты поиска: Описаны различные ситуационные проблемы, с которыми сталкиваются ревматологи в оказании помощи женщинам репродуктивной возрастной группы, заинтересованным в создании семьи. Пересматриваются современные решения, основанные на фактических данных, в которых особое внимание уделяется безопасному использованию противоревматических препаратов, модифицирующих болезнь, и модификаторам биологического ответа для оказания помощи ревматологам в уходе за беременными и кормящими женщинами с РА.
Заключение: К сожалению, вопрос фертильности у больных РА сравнительно мало изучен. При этом женщины с РА имеют более высокий риск неблагоприятных исходов беременности, и, следовательно, необходимо проводить тщательный дородовой и послеродовый мониторинг с целью уменьшения осложнений. Вынашиванию беременности должны предшествовать тщательное планирование с учетом фармакотерапии и полная клиническая ремиссия ревматоидного артрита.
Необходимы дальнейшие исследования, чтобы изучить эти данные в отношении тяжести заболевания, используемых лекарств и наличия других сопутствующих заболеваний.
