ИЗУЧЕНИЕ ВЛИЯНИЯ ТИННИТУС НА ПСИХОЛОГИЧЕСКОЕ СОСТОЯНИЕ ПАЦИЕНТА. ОБЗОР ЛИТЕРАТУРЫ
Введение: Тиннитус - это восприятие звука при отсутствии каких-либо внешних раздражителей. Симптомы могут быть односторонними или двусторонними, присутствовать с потерей слуха или без него и напоминать звон, шипение, свист, гудение, жужжание, щебетание или щелчки. Однако до сих пор отсутствует единый консенсус по методам диагностики и лечения пациентов с тиннитус.
Цель: обзор литературы о влиянии эффективных методов лечения на психологическое состояние пациентов при тиннитус.
Стратегия поиска: Поиск литературы проводился в базах данных: PubMed, the Cochrane Library. Стратегия поиска включала ключевые слова: tinnitus, quality of life, efficiency or effectiveness, psychology, distress, anxiety, treatment, neurostim*, masking, therapy, music. Методологические фильтры включали систематические обзоры и метаанализ. Поиск был ограничен английским и русским языками, опубликованными в период с января 2000 года по сентябрь 2021 года (имеется 6 литературных источников 1983,1990,1991,1995,1996 годов). Три автора независимо друг от друга проверили все отобранные работы. Исследование оценивалось по контрольному списку AMSTAR-2. Критерии включения: обсервационные исследования, включая перекрестные исследования, разные диагностические критерии, разные возрастные группы, разную направленность исследования и различия в отчетности и анализе результатов. Критерии исключения: статьи и материалы, не имеющие доказательной базы, резюме докладов, тезисов и газетные статьи, материалы конференции и публикации низкого методологического качества, которые не отражали основной значимости, с не ясными выводами. В результате отбора были проанализированы 137 публикаций по данной теме, из них 59 статей прошли критический анализ оценки.
Результаты и выводы: Авторы систематических обзоров и клинических руководств отмечают, что тиннитус влияет на психологическое состояние пациента, и когнитивно-поведенческая терапия является одним из эффективных методов лечения. Также применение опросников для выявления уровня тревожности и депрессии важно для исключения психологических расстройств у пациентов с тиннитус.
