ОЦЕНКА КАЧЕСТВА ЖИЗНИ ПАЦИЕНТОВ С ОПУХОЛЯМИ ГОЛОВЫ И ШЕИ С ИСПОЛЬЗОВАНИЕМ ОПРОСНИКОВ НА ФОНЕ СПЕЦИАЛИЗИРОВАННОГО ЛЕЧЕНИЯ
Введение: Пациенты со злокачественными опухолями головы и шеи (ОГШ) нуждаются в сохранении качества жизни, применение любого из доступных методов лечения характеризуется агрессивностью и может приводить к ухудшению функции органов, отвечающих за речь, прием пищи и дыхание, с последующим снижением качества жизни в виде дисфагий, затруднённого дыхания, нарушения глотания, появления косметических изъянов лица и шеи, ксеростомии, тризм, потери слуха, остеонекроза нижней челюсти, которые являются частыми симптомами после облучения, все это не может не влиять на благополучие пациента во всех сферах человеческой жизни [1], [13]. В Казахстане существует пробел в изучении КЖ пациентов с раком головы и шеи, отсутствуют исследования, изучающие связь между предикторами КЖ и выживаемостью, настоящая работа имеет цель хотя бы в малой степени восполнить этот пробел и привлечь внимание к данной проблеме.
Цель: проанализировать статьи с исследованием КЖ пациентов на фоне специализированного лечения.
Стратегия поиска: проведен автоматизированный поиск релевантных источников, по ключевым словам качество жизни, опухоли головы и шеи, анкетирование, лучевая терапия в научных базах PubMed, а также ручной поиск в системе Google Scholar. Глубина поиска составила 10 лет (с 2011 по 2021 г). Источники отбирались в соответствии с базовым контекстом исследования. Из 56 статей было выявлено 5 опросников для оценки КЖ пациентов с ОГШ. Анализ источников и выделение информационных блоков, впоследствии использованных в написании обзора.
Критерии включения: не было строго задано критериев включения материалов в литературный обзор, кроме использования материалов по контексту и по экспертному анализу исследователя, так как мы не имели полного доступа к базам данных платной литературы, а также к рукописям, находящимся в частичном доступе.
Результаты: Анализ анкет (FACT-H&N, UW-QOL, EORTC QLQ-C30/QLQ-H&N35/43) представил статистически значимую корреляцию КЖ и побочных симптомов лечения [3].
Выводы: Тщательный мониторинг оценки КЖ позволяет медицинскому персоналу более точно представить эффект выбранного лечебного воздействия и сформировать группы пациентов с риском прогрессирования, в итоге может использоваться в качестве нового прогностического маркера выживаемости.
