КЛИНИЧЕСКОЕ ПРИМЕНЕНИЕ ДЕКСМЕДЕТОМИДИНА (ДЕКСДОРА) ПРИ ДЕЛИРИЯХ И ПСИХОЗАХ В ОТДЕЛЕНИИ АНЕСТЕЗИОЛОГИИ, РЕАНИМАТОЛОГИИ И ИНТЕНСИВНОЙ ТЕРАПИИ. ОБЗОР ЛИТЕРАТУРЫ.
Введение. Адекватная седация пациентов в отделениях реанимации и интенсивной терапии – одна из наиболее актуальных проблем современной анестезиологии и реаниматологии. Делирий в отделении интенсивной терапии поражает примерно 30% пациентов, несмотря на использование эффективных профилактических стратегий. Стратегия поиска. Нами был проведен литературный поиск в базах данных PubMed, Web of Science, e-Library, Кокрановском центральном регистре контролируемых испытаний. Для поиска были использованы ключевые слова: «делирий», «отделение интенсивной терапии», «дексмедетомидин», «дексдор». Поиск выявил 924 публикаций. Текущий обзор включает 43 публикации. Результаты. Настоящий обзор включает обзор патофизиологии и типов делирия, описывает методы, используемые для скрининга делирия. Также обсуждаются опубликованные исследования, связанные с использованием Дексмедетомидина для лечения делирия. Авторы дают рекомендации персоналу отделений интенсивной терапии по использованию Дексмедетомидина в контексте оказания доказательной помощи пациентам отделений интенсивной терапии с делирием. Заключение. Методика седации с использованием Дексмедетомидина обеспечивает высокий уровень комфорта и возможность активизации пациентов с делирием, не подавляет самостоятельное дыхание и не вызывает значительных побочных эффектов.
