ЭНДОСКОПИЧЕСКОЕ ЛЕЧЕНИЕ КРОВОТЕЧЕНИЙ ВЕРХНИХ ОТДЕЛОВ ЖЕЛУДОЧНО-КИШЕЧНОГО ТРАКТА. ОБЗОРЛИТЕРАТУРЫ.
В литературном обзоре представлены вопросы эндоскопического лечения желудочно-кишечных кровотечений из верхних отделов пищеварительного тракта. Обоснованы выбор, стратегия и методики при различных клинических ситуациях, в том числе и при рецидивирующих кровотечениях. Повествуется о гемостатических устройствах и препаратах, о преимуществах разных видов клипс, применяемых с целью гемостаза.

Актуальность. Острые желудочно-кишечные кровотечения остаются актуальной проблемой неотложной абдоминальной хирургии. В большинстве случаев желудочно-кишечное кровотечение останавливается спонтанно. В других случаях требуется то или иное вмешательство. Выбор метода лечения зависит от источника кровотечения. Эндоскопические процедуры являются эффективными методами остановки кровотечения, позволяющими избежать обширных лапаротомных оперативных вмешательств. У хирурга должен быть выбор между различными методами гемостаза, основанный на конкретных условиях работы.
Цель. Анализ проблем о лечении верхних отделов желудочно-кишечных кровотечений, о своевременности назначения медикаментозной и эндоскопической гемостатической терапии по данным современной литературы
Стратегия поиска. В исследовании изучены полнотекстовые публикации на английском и русском языках, которые посвящены эндоскопического лечения желудочно-кишечных кровотечений из верхних отделов пищеварительного тракта в зарубежных странах. В процессе поиска литературы использованы следующие поисковые системы: Pubmed, Web of science, Cyberleninka, Google Scholar по ключевым словам. Изучались и анализировались публикации с 2009 по 2022 годы, статьи в основном были зарубежные и на английском языке. По данной теме выявлено более 130 публикаций. Из них цели нашего исследования соответствовало 86 публикаций. Критерии включения: Публикации уровня доказательности А, В: мета-анализы,систематические обзоры, когортные и поперечные исследования. Критерии исключения: краткие отчеты, газетные статьи и личные сообщения.
Результаты и выводы. В литературном обзоре представлены вопросы эндоскопического лечения желудочно-кишечных кровотечений из верхних отделов пищеварительного тракта. Обоснованы выбор, стратегия и методики при различных клинических ситуациях, в том числе и при рецидивирующих кровотечениях. Повествуется о гемостатических устройствах и препаратах, о преимуществах разных видов клипс, применяемых с целью гемостаза.
