ДИАГНОСТИЧЕСКАЯ И ТЕРАПЕВТИЧЕСКАЯ ЛАПАРОСКОПИЯ ПРИ ТРАВМАХ ЖИВОТА. ОБЗОР ЛИТЕРАТУРЫ.
Введение. У пациентов травмы живота связаны с серьезными осложнениями, ведущими к летальному исходу. Частота проникающих травм сильно различается по всему миру. Различается и мировой опыт лапароскопии у пациентов с травмами. Исследователями доказано, что лапароскопия играет все более значимую роль у пациентов с травмами живота.
Цель. Анализ литературы, отражающей актуальные сведения о диагностической и терапевтической лапароскопии при травмах живота.
Стратегия поиска. В данное исследование были включены публикации, преимущество на английском языке, взятые для анализа из наиболее актуальных баз данных, таких как: PubMed, Embase, Scopus, Web of Science Core Collection. Ключевые запросы включали следующие термины: диагностическая лапароскопия, терапевтическая лапароскопия, диагностическая лапароскопия при травмах живота, терапевтическая лапароскопия при травмах живота, далее подходы к травмам живота и сравнение лапаротомических и лапароскопических операций при травмах живота. Глубина поиска составила преимущественно до 30 лет, так как тема редкая, источников последних лет не так много, поэтому были рассмотрены работы и более ранних лет, имеющих значительную научную ценность. Было найдено 102 статьи, из них для анализа было отобрано 71 статья.
Результаты и выводы. Представлены современные вопросы лапароскопии при травмах живота. Раскрыты подходы и стратегия выбора между лапароскопией и лапаротомией. Диагностическая и терапевтическая ценность лапароскопии представлена в виде различных клинических результатов. Сравнение результатов лечения между лапароскопией и лапаротомией в послеоперационном периоде по ряду показателей – летальности, осложнения, пребывание в стационаре. Лапароскопия стала методом выбора при многих острых хирургических состояниях. С годами растет число и значимость лапароскопических операций. Однако необходимы более тщательно спланированные рандомизированные клинические исследования, чтобы подтвердить ценность лапароскопии для диагностики и лечения травм живота.
