ЖАМБАСТЫҢ АРТҚЫ ЖАРТЫСАҚИНАСЫНЫН ТҰРАҚСЫЗ ЖАРАҚАТТАРЫНЫҢ ХИРУРГИЯЛЫҚ ЕМІ. ӘДЕБИЕТТІК ШОЛУ
Кіріспе. Жамбас сақинасы жарақатынын әртүрлілігі қазіргі таңдағы травматологиянын күрделі және өзекті мәселесі ғана болып табылмайды, сонымен қатар күрделі әлеуметтік мәселе болып келеді. Жамбас сақинасының сынықтары мен жарақаттары салыстырмалы түрде сирек кездеседі, бірақ жарақаттанулардың өсуіне байланысты олардың саны да көбеюде. Оның емі ортопедиялық хирургиянын ең қиын жұмыстарының бірі болып табылады. Отадан кейінгі алыс кезеңде емнің нашар нәтижелері 30 дан 60 пайызға дейін болуы мүмкін. Жоғарыда көрсетілген жағдайды ескере отырып, жамбас жарақаты және онын емі әлі де терең зерттеуді талап ететінін айта кету керек.
Зерттеудің мақсаты. Жамбастың артқы жартысақинасының тұрақсыз сынықтарының хирургиялық емі туралы әдебиеттерді сараптау.
Зерттеу әдістері. Ғылыми ақпаратты іздеу Google Scholar, PubMed, e-library жүйелерінде және «қолмен іздеу» әдісі арқылы өткізілді. Іздеудің келесі стратегиясы қолданылды PubMed (MeSH Terms: “posterior pelvic ring, sacrum, sacroiliac joint, fractures, surgical treatment”). Іздеу тереңдігі 10 жыл, 2009 дан 2019 жылдар. Қосу критериялары: толық мәтінді жазбалар, клиникалық рандомизацияланған және рандомизацияланбаған зерттеулер. Шығару өлшемдері: баяндама резюмесі, газет жарияланымдары, жеке хабарламалар. Барлығы 2600-ден астам әдебиет табылып, оның 97-сі сараптауға алынды.
Нәтижелері. Жамбас сүйектері зақымдалуларын емдеудің заманауи тәсілі хирургиялық ем және ерте функционалды-тұрақты остеосинтез болып саналады, және оның негізгі мақсаты жамбас сүйектерін қалпына келтіру және тұрақтандыру. Кейбір ресейлік авторлар жамбас сүйектері емін сырттан бекіту тәсілдерімен жасағанды қолдаса батыс елдері авторлары ондай зақымдалулуарды функционалдыө тұрақты іштен бекіту әдістерін таңдайды. Қазіргі таңда жамбастың артқы құрылымдарының зақымдалуларын хирургиялық бекітудің бірнеше әдісі бар екенін және олардың тек жақсы нәтиже ғана емес әртүрлі асқынулары бар екенін де айтқан жөн.
Қорытынды. Жоғарыда айтылғандарға сай жамбас сүйектері зақымдалуларын және оны емдеу әдістерін терең зерттеулерді жалғастыру қажет. Жамбас сүйектері мен байламдарын хирургиялық бектітетін құрылғылар мен импленттарды жобалаудың маңызы сақталуда және олардың қауіпсіздігі мен клиникалық пайдасын бағалау үшін проспективті клиникалық зерттеулер жүргізілуі қажет.
