ЛЕГГ-КАЛЬВЕ-ПЕРТЕС АУРУЫНЫҢ ХИРУРГИЯЛЫҚ ЕМ ТӘЖІРИБЕСІ
Кіріспе.Мақала барысында Пертес ауруының фрагментация дәрежесіндегі емдеу нәтижесі сипатталған. Емдеудің негізгі мақсаты сансуйегі басының пішіні мен құрылымының қалыптасуына максимальді мүмкіндік туғызу. Берілген стержендік сыртқы бекіту құрығысы мен Пертес ауруының классикалық және ауыр түрлерін емдеудің альтернативті белгілі хирургиялық әдістерімен қатар қолдануға болады.
Мақсаты. Балалардағы Пертес ауруының хирургиялық ем нәтижелерін жақсарту.
Материалдар мен әдістер. Берілген зерттеудің дизайны тәжірибеде болған жағдайды сипаттау. Нұр-Сұлтанқаласының №2 қалалық балалар ауруханасында, балалар ортопедиясы және реабилитация бөлімінде орындалған Пертес ауруының фрагментация сатысындағы емнің ерте және кеш нәтижелері бақыланды. Пациенттің жасы 11 жас 4 ай, ер бала. Хирургиялық іс-шараға бізбен құрастырылған технологиямен сыртқы фиксациялық стерженьдік аппаратының монтажымен орындалады, онда дұрыс дистракцияға арналған мөлшерлік жүйесі бар және телескоптық стерженьдердің көмегімен орындалады. Элементтерді ұстап–бекіту үшін Илизаров аппаратының жартылай доғаларына сүйендік.
Результаты. Бақылау уакыты 15 ай болды. Буында орындалатын толыққанды қозғалыстарды 2,5 айда қолжеткіздік. Ауырсыну синдромы болған жоқ. Сан сүйегі басының пішінімен құрылымының жартылай қалпына келуі операциядан кейін бірінші 6 айында байқала басталды. Соңғы қарау кезінде эпифизарлы индекс көрсеткіші 0,4 құрады. МДБ мөлшері 135° болды. Шентон сызығының үздіксіздігі сақталды.
Рентгенологиялық нәтижелері Herring критериімен бағаланды: В тобы. Буын Stulberg бойынша II классқасәйкескелді.
Қорытынды. Берілген аппарат жамбас-сан буынын ауырсынусыз орындауға мүмкіндік береді. Сол кезде қанайналым жақсарады, жұмсақ тіндердің және сан сүйегінің басының қорегі қамтамасыз етіледі, ем барысында сансүйегінің қалыптасуының мерзімдерін азайтады.
