ОСОБЕННОСТИ РЕСПИРАТОРНОЙ ПОДДЕРЖКИ ПАЦИЕНТОВ С ПНЕВМОНИЕЙ В УСЛОВИЯХ ПАНДЕМИИ COVID-19
Введение. Пандемия коронавирусной инфекции (COVID-19) на пике активности. С начала пандемии понимание нозологии растет с каждым днем. Хотя на сегодняшний день ведутся крупные рандомизированные исследования, лечение основывается на поддерживающей терапии. Текущие рекомендации основываются на лечении других вирусных пневмоний и сепсиса. Но течение COVID-19 в значительной степени отличается от них.
Цель – анализ статей по теме респираторной поддержки коронавирусной инфекции COVID-19, опубликованных в наиболее авторитетных источниках с доказательной базой.
Стратегия поиска: поиск осуществлялся в следующих базах - PubMed, Google Scholar, Scopus, Embase. Начальное количество статей - 325 (в том числе повторяющиеся статьи - 188), изучены названия и абстракты статей - 137 (в том числе 63 статьи исключены по следующим причинам – отсутствие группы сравнения, несоответствие целей и названия статей). Полностью изученные статьи - 74 (в том числе 39 статей исключены по следующим причинам – отсутствие рандомизации пациентов, наличие методологических ошибок). Количество статей вошедших в исследование – 35.
Результаты. Основная задача в лечении острой дыхательной недостаточности – адекватная оксигенация, так как гипоксия может привести к тяжелым и непоправимым функциональным нарушениям. У пациентов с COVID-19 оксигенотерапия и респираторная поддержка должна начинаться до поступления в реанимацию. Самым доступным в терапии ОДН является оксигенотерапия.
Выводы. Пандемия COVID-19 требует расширенного и всестороннего анализа рисков и пользы проведения респираторной поддержки, в том числе НИВЛ и ИВЛ. Респираторная поддержка современными вариантами оксигенотерапии и НИВЛ потребует тщательной оценки рисков заражения медицинских работников, внутрибольничного распространения инфекции, наличия ресурсов здравоохранения и клинического спектра заболеваний у коморбидных пациентов с COVID-19.
