ҚАЗАҚСТАНДАҒЫ СҮТ БЕЗІ ОБЫРЫ КЕЗІНДЕГІ ОНКОЛОГИЯЛЫҚ ҚЫЗМЕТ КӨРСЕТКІШТЕРІНЕ COVID-19 ӘСЕРІ
Кіріспе. Қатерлі ісік ауруын зерттеу жөніндегі халықаралық агенттіктің болжамына сәйкес, 2040 жылы сүт безі қатерлі ісігінің (СБҚІ) 3 миллионға жуық жаңа жағдайы болжанып, осы патологиядан 1 миллионға жуық әйел қайтыс болады деп күтілуде. Зерттеу мақсаты: Қазақстандағы сүт безі қатерлі ісігінің жанындағы онкологиялық қызметтің көрсеткіштеріне COVID-19 әсерін бағалау. Зерттеу материалдары мен әдістері. Зерттеу материалы Қазақстан Республикасы Денсаулық сақтау министрлігінің 2011-2020 жж. арналған РМЖ (АХЖ 10 – С50) қатысты жылдық №7 және №35 нысаны болды. Негізгі әдіс ретінде биомедициналық статистиканың сипаттамалық және аналитикалық әдістерін қолдана отырып, ретроспективті зерттеу қолданылды. Нәтижелері. 2011-2020 жылдары республикада алғаш рет 42 376 жаңа СБҚІ жағдайы және осы патологиядан 12 914 өлім тіркелді. СБҚІ-мен сырқаттанушылықтың орташа жылдық өрескел көрсеткіші 46,4±1,10/0000 (95% ДИ=44,3-48,5) құрады, динамикадағы көрсеткіш 40,8±0,70/0000 (2011 ж.)-ден 2019 жылы 51,3±0,70/0000-ға дейін өсті және одан әрі 2020 жылы 44,2±0,70/0000-ға дейін төмендеу байқалды (р<0,001). Серпінінде СБҚІ өлім-жітім көрсеткіштері статистикалық маңызды (р<0,001) төмендеу үрдісіне ие болды, 16,1±0,40/0000 (2011 ж.)-дан 11,9±0,40/0000-ға (2019 ж.) дейін және 2020 жылы 11,5±0,40/0000-ға дейін, ал СБҚІ өлім-жітімінің орташа жылдық өрескел көрсеткіші 14,2±0,60/0000 құрады (95% ДИ=13,1-15,3). Ерте диагностика көрсеткіштері (I-II сатысы бар науқастардың үлес салмағы) 73,4%-дан (2011 ж.) 86,9% - ға (2019 ж.) дейін жақсарды және 2020 жылда 84,2%-ға дейін төмендеді. III сатыдағы науқастардың үлес салмағы төмендеді (2011 жылғы 20,9%-дан 2019 жылы 8,4%-ға дейін) және IV сатыда (тиісінше 5,6%-дан 4,2%-ға дейін), яғни елеусіздік көрсеткіштері төмендейді. СБҚІ кезінде морфологиялық верификация көрсеткіштері әрдайым жоғары болды және зерттелген жылдар ішінде 95,7%-дан 99,4%-ға дейін жақсарды. Тұжырымдар. СБҚІ жанындағы онкологиялық қызмет көрсеткіштерін талдау нәтижесінде морфологиялық верификация және ерте диагностика көрсеткіштерін жақсарту, үдерістің қараусыздығы мен өлім көрсеткіштерін төмендету анықталды. Алайда, 2011-2019 жылдардағы тенденциялар 2011-2020 жж.кезеңдерінен айтарлықтай ерекшеленді, бұл COVID-19 пандемиясының СБҚІ ағымына, есебіне және тіркеуіне әсер етуімен байланысты.
